For copies of reports produced form origin and cause investigations:

CD's produced with investigation reports, incident reports, photographs, diagrams, witness statements, and other documents relating to a specific incident will cost $25.00 each.  Any reports, or parts of an investigation report, generated outside our department will be charged at cost.  All additional paper reports will be charged at .25 cents per page.
